  • 目前排名 / WAGR/JGS Ranking（公開組必填/for Open Division）
  • 如無排名請提供1－3場比賽成績/If you do not have ranking, please provide scores of tournaments.
  • 是否參加4月20日練習賽 / Participating practice round
  • 是否參加歡迎晚宴 / Participating Welcome Party
  • 飲食需求 / Dietary Preference
  • 肖像權 / Portrait Rights（在比賽期間的選手肖像權是屬於大會的，大會得以使用於相關賽事宣傳。 The player portrait rights during the competition belong to the tournament, and it can be used for the promotion of the tournament related events.）
